How to Get your Medical Document

Get your Medical Cannabis Document by following these three easy steps.

Complete patient intake and consent form

In this step, you complete the patient intake form and consent form on our home page.

Pay the consultaiton fee

In this step, you pay the consultation fee on our payment page.

You get your medical cannabis document 

Within 48 hours of submitting your patient intake and consent form and paying your consultation fee, you will receive an email confirming that your medical document has been submitted and receiving a treatment plan for medical cannabis.
